I the undersigned, hereby consent to allow Team Keto, LLC to use my before and after digital photographs, measurements, written and/or audio/video testimonial and grant all rights to any digital recordings of me in the possession of Team Keto, LLC including all rights to copy, exhibit, publish, or distribute the works in print and electronic form, publicly or privately, and to market and sell copies and to use these in any and all types of media, now or hereafter known, for the purpose of marketing and promotions. I agree that I will make no monetary or other claim against Team Keto, LLC for the use of my digital photographs, measurements, written testimonial and audio/video testimonial. I waive any rights, claims, or interest I may have to control the use of my identity or likeness in whatever media used except as provided herein. I waive any right to inspect or approve the finished product, including written copy, wherein my likeness or my testimonial appears. I represent that I am at least 18 years of age, have read and understand the foregoing, and am competent to execute this agreement. I hereby hold harmless and release Team Keto, LLC from all claims, demands and causes of action which I, my heirs, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization I have read the authorization and release information and give my consent for the use as indicated above.
